I did initial assessments, collaborated with patients on discharge planning, interviewed family members to obtain collateral information, contacted nursing homes for information concerning referrals, wrote progress notes, and filled out PAS/RR forms for discharge planning. Also, as a project, I was able to update information on medical equipment providers in Ohio, Kentucky, and West Virginia that allowed me to present, not only contact information, but information about the specific equipment each provider carried.
