Case Manager
-
Nov 2020 - Nov 2021
Onsite Case Manager at Miriam's Kitchen. I help support clients with obtaining documents such as their ID, Birth Certificate, and Social Security Card. I also help clients apply for income benefits and health insurance. I also support clients with their housing processes such as connecting them with their housing providers and submitting paperwork. My daily operations include providing clients with tolitries, mail, voicemail messages, meals, clothing, and other things that they need and are available. Case Managers also help provide a safe environment in which clients are able to interact. We've been heavily involved in referring clients to PEP-V and coordinating transportation to those sites and also utilizing the DHS COVID-19 hotline to connect guests to the ISAQ sites. We also work to connect clients to mental health agencies and medical care that can support them and address their needs.
