I collaborate with patients, families, and members of the healthcare team by assessing, planning, implementing, coordinating, monitoring and evaluating the options and services required to meet the patient’s health care needs during hospitalization and for discharge into the community. In addition, I have a focus on denial management, maintaining denial logs and following through the denial process until either a favorable outcome is achieved or the denial process is exhausted.
