Anna Freeman
During my two and a half years of employment at Project Open Hand (POH) in Oakland, CA, I had the privilege of working alongside Jenna as her colleague and site manager. In my experience, serving on the POH Oakland team requires one to be highly adaptable, collaborative, and—most importantly—empathetic towards the diverse group of clients, many of whom come from traditionally marginalized communities and experience disproportionate health disparities. In her work as the Registered Dietician for POH Oakland, Jenna sets an example for other staff and volunteers on how to function successfully in a high-stress environment by demonstrating these characteristics daily. I continually observed Jenna fostering relationships with her clients that supported and empowered them to adopt positive behavior change. When conducting quarterly and bi-annual program evaluations, I was in constant awe of the feedback that clients gave about their experience working with Jenna and the impact that she had on their lives. This feedback speaks directly to Jenna's genuine compassion, patience, and knack for meeting people where they are. What impressed me most about Jenna was her ability to stay abreast of the changing information and best practices required to perform her duties. Without direct contact to her supervisor and fellow dieticians who are based out of POH’s San Francisco office, Jenna manages to independently learn and grow in her role as Registered Dietician. Clients come to POH with an array of chronic diseases, including but not limited to clients living with HIV, various cancers, heart disease, and diabetes. Jenna is instrumental in ensuring the upmost integrity and quality of services delivered to all clients, while providing individualized support based on each person’s specific health needs. Above all, Jenna is humble, thoughtful, and committed to self-reflection and personal growth.

At this nonprofit, I provide Medical Nutrition Therapy (MNT) to patients living with HIV, diabetes, cardiovascular disease, and other chronic illnesses. The vast majority come from medically underserved populations. MNT is an evidence based approach to disease management that includes nutrition assessment, diagnosis, counseling, and monitoring. As part of MNT, I use hands-on Subjective Global Assessment to assess my patients' nutritional status, and motivational interviewing to help them form achievable, measurable goals and plans. Lately I have been collaborating with colleagues around the state to implement the first-of-its-kind Medi-Cal Medically Tailored Meals (MTM) Pilot Program. The program’s aim is to lower re-hospitalization rates among people living with congestive heart failure. I provide four MNT sessions, including two in the patient’s home. Over 85% of my patients are people of color and 44% self-identify as LGBTQI+, so I have considerable experience providing culturally competent nutrition assessment and counseling. I also advise and collaborate on the design and production of medically tailored meals for all of the agency’s diverse programs. I have developed dozens of patient nutrition education handouts, cookbooks, and cooking classes to help my patients learn how to use the nutritious groceries we provide in our wellness-oriented nutrition centers. Using hands-on Subjective Global Assessment, motivational interviewing, and other tools, I provided Medical Nutrition Therapy (MNT) to all units of the hospital. I collaborated in interdisciplinary care rounds in the adult ICU, Neonatal ICU, and on-site Skilled Nursing Facility. During my time at Sequoia, I demonstrated my expertise in enteral and parenteral nutrition support by achieving certification as a Certified Nutrition Support Clinician (CNSC). Besides providing inpatient nutrition education, I also collaborated with my colleagues to develop and implement an outpatient nutrition program. In addition, I taught the nutrition portion of the hospital’s cardiac and pulmonary rehabilitation programs. Show less
