I managed a caseload of up to twenty adult clients who are diagnosed with Schizophrenia; many dualy diagnosed. In doing so I taught daily independent living skills, accompanied/assisted with team of doctor appointments, coached on symptom management techniques, supervised on medication regiments, and collaborated with facilities like: hospitals, skilled nursing facilities, psychiatric emergency, IMD's, and County case managers. Additionally, I work closely with other agencies throughout Marin to best serve each clients multiple needs (i.e. Buckelew Employment Services, STAR team, Community Action Marin, Enterprise Resource Center, Ritter Center, MCC, Department of Rehab, Novato Food Bank, etc...). I did a lot of crisis intervention and aftercare work in order to ensure clients continued to reside in our supported and independent housing.
