My role as a residential aged care pharmacist involved providing the doctors and registered nurses with RMMR(Residential medication management reviews) reports. Recommendations made in RMMR reports identified critical items in medication regimens, improving health outcomes. I was also resident member of Internal and corporate MAC meetings across many aged care facilities in Sydney working closely with Doctor’s, Facility Managers, and other key stakeholders. Provide Quality use of medicines (QUM) facility-focussed services such as implementing and monitoring policies, activities for the safe and effective prescribing and administration of medicines. This included facilitating in-service training sessions on topics including S8 management, polypharmacy and the use of psychotropics. Providing aged care facility with regular monthly review reports of psychotropics used for behavioural disturbance in dementia patients, as per Royal commission into Aged care guidelines. Prepare comprehensive medication audit discrepancy reports which served as a key tool for maintaining medication administration accuracy.
