My current role I am a housing support worker my roles focus on supporting the clients after they have been in the project for over three to six months and are ready to be supported back into the community in the way of education, employment or volunteering Identify users individual support needs and, through consultation and agreement with the user (and, where appropriate, their Care Manager), devise and implement suitable support plans to ensure that identified support needs are met. As Offender resettlement Manager my main duties are to interview offenders in the first Ten days of coming into custody to carry out an assessment that outline the kinds of support that the offender will need during their time in custody ensure services promote independence and inclusion for clients, whilst following all forms of policies and procedures, gain their confidence and motivate them into education, training, or employment inside custody. After collecting the information I would sign post to other agencies including probation communicating with them in many forms on P Nomis, OAsys, emails and face to face building strong relationships making communicating easier and more streamline. Three months prior to their release I will do a more in depth review of the offenders progress prioritising offender’s into order of release and sign post to housing, Job centre plus, National career’s advice, mentoring organizations like SOVA and trailblazers, ensure that the offenders have I.D and are in communication with probation. I will provide disclosure advice and ensure that the offender is in the best place to be re-introduced into the community I have a large case load that I am responsible for and are outcome base targeted.
