  Linkage Care has been founded by a team of aged care experts, motivated to make a positive influence on the lives of older Australians. Our team of experienced Registered Nurses work closely with families to find the best care solution for their loved one. We only recommend solutions that allow a loved one to continue to lead a fulfilling and independent life. We don’t believe anyone should have to give this up. Sometimes a solution can be achieved by bringing extra support into the home. Sometimes a new, more supportive care environment is needed. Linkage Care is based on meeting three critical consumer needs (1) to have peace of mind they are receiving the right type of care and support from quality providers (2) peace of mind they can continue to lead a fulfilling life, from their own home or a supportive community of their choice and (3) peace of mind their care will be adjusted as their needs change. Linkage Care helps assess, plan and then finally connect the consumer to care providers and other relevant specialist service providers as needed. We have a team of registered nurses who come to a client’s home to assess care needs, as well as the government support they may be eligible for. We then prepare a Linkage Care Plan which outlines a combination of care and lifestyle services that will deliver the best possible quality of life. Linkage Care then puts the client in touch with a range of high quality provider who can deliver the care and lifestyle services needed.
