  - Provide wrap-around social, employment and educational supports through relationships built with employers, program providers, and community partners to meet the needs of clients- Address client's concerns through interviews by phone and in person at various locations such as work-from-home, offices, community centers and make the appropriate referrals to services and resources required including employers, Ontario Works, food banks, clothing banks and shelters- Monitor and evaluate programs, execute needs assessments, and provide appropriate reports on all programs, intakes, caseloads, and services while maintaining client confidentiality- Collect, evaluate, and verify clients' documents and information to determine eligibility for appropriate programs, income supports, career pathways, and referrals- Research and establish relationships with agencies to expand range of programming and service opportunities for client referrals- Develop support plans for clients including follow-ups and alternative opportunities due to program ineligibility or changes in client circumstances- Implement detailed plans and recommend policies/procedures regarding program specific requirements- Co-ordinate and provide professional advice regarding career development and support to clients- Ensure the development and implementation of effective strategies and address future client needs and improve rapport with partnering organizations
