The Personal Helpers & Mentors Program assists people whose ability to manage their daily activities and live independently in the community is severely impacted by their mental illness. Participants are supported through a recovery-focused, strength-based approach that recognizes recovery as a personal journey driven by the participant. As Team Leader I ensure appropriate allocation of participants to case workers. I monitor activities and provide leadership to ensure staff are actively working with participants to formulate and review their individual recovery plans as well as maintaining accurate records and reports in accordance with requirements from our funding body and our organisation. I provide strength-based supervision on a regular basis promoting ethical and non-discriminatory behaviours and practices which incorporate respect, dignity confidentiality and a duty of care. As Case Worker I provided support, advocacy and mentoring for a caseload of 12 participants. I worked collaboratively with participants to identify, develop and implement their individual recovery plans. I maintained knowledge of resources and supports available for consumers with multiple and complex needs and made referrals as appropriate.
