Within my role as a Family Support Worker I held a caseload of around 30 families who have a child or young person with a life-limiting or life threatening illness or disability. I was required to recognise the needs of the family when meeting with them and to have clear lines of communication to enable effective communication through a professionally boundaried relationship. I was required to work autonomously, managing my time and my caseload, I was required to prioritise visits and meetings, always following the charity's guidelines and policies. I kept accurate and up to date notes of all communication with families and professionals. I regularly attended Multi Disciplinary Team meetings and social services meeting's, I was required to attend and undertake part of an initial assessment with families when they are referred to the service - if they did not meet our criteria i would often undertake small pieces of o o sign post them on to more appropriate services. As well as case work I, along with my colleague made a Sibling Support Programme to deliver to the siblings of our service users. I was required to undertake research into what our families would like to gain from this and make a programme to incorporate these wishes along with other research that we undertook I was required to have a good knowledge of child protection and safeguarding both locally and nationally. I was required to arrange and chair Team Around the Child meetings - these meetings enabled us to listen to any concerns that parents have and to deliver a coordinated approach. Participating in Multi-agency developmental assessments, attending regular referral & caseload meetings, coordinating services for families, home visits- to establish how the family were coping and assessing if there was anything that we could further support with i.e, continence assessments or sign posting on to other services, I would also attend social services Children In Need meetings. I regularly attended supervision, both group and individual. I was required to work autonomously, managing my time and my caseload, prioritising visits and meetings, always following the trust guidelines and policies.
