      I worked as a part of Lowell General Hospital’s Fallon Wellforce ACO Care Complex team to work with Fallon’s high risk population. I advocated and worked collaboratively with clients as well as their respective Circle Health Primary Care Providers, community care partners, outpatient care providers, and family members to make sure that clients needs are met with the ultimate goal for them to stay well in the community, increasing their quality of life. I attended daily interdisciplinary meetings with ACO Complex team, and bi-weekly meetings with community partners. I created feasible care plans for clients according to their needs and review them with clients regularly. I delegated follow-up tasks when needed to other interdisciplinary team members to ensure clients’ needs are met. I conducted multiple assessments including comprehensive, PHQ-9, as well as transition of care assessments, and make appropriate referrals for the clients to community and/or state agencies while clients are outpatient. I conducted consistent follow-up calls and provide brief-interventions when necessary, and home as well as community visits with clients when appropriate. While clients are inpatient, I continued to follow-up on their cases with inpatient staff and follow-up with clients after discharge to ensure complete connected care. Show less
