Project Manager, Digital Out-of-Home Care Project, Integrated Care Team
-
Jan 2017 - Dec 2019
Project Management of the Integrated Care Innovation Fund Out-of-Home Care project working to build shared care and digital information-sharing pathways between GPs, specialists and Hospitals and Health services. This project targeted improving health care for children and young people in out-of-home-care by developing a suite of digital clinical assessment tools for use by GPs using their software platforms - Medical Director & Best Practice. Other deliverables included an educational video, social media campaign and development of a Health Pathway for use by GPs. Strong stakeholder engagement was key to the success of this project, and I successfully managed partnerships between Dept of Child Safety, Youth and Women, Primary Health Networks, and ATSICHS to strengthen our outcomes. https://www.childrens.health.qld.gov.au/chq/health-professionals/out-of-home-care/
