We work closely with the therapy department, and make necessary referrals if we notice a decline in our residents. I coordinate Dining skills committee, in which we discuss dietary concerns for our residents. This includes, weight loss, weight gain, residents with wounds, residents that need dietary consistency changes, and dining/eating concerns in general. I receive fall reports for our facility. It is my job to analyze any possible root cause of falls (medications, infections, proper footwear, decline in function, etc), work with interdisciplinary team to provide interventions to prevent further falls.
