-Gathers and confirms patient information prior to a patient’s scheduled appointment so providers have all the necessary information in the electronic health record prior to patient appointment.-Call new patients to educate on expectations of first visit and confirm appointment date/time.-Obtain medical summary to upload into patient’s chart for review by the Nura new patient care team.-Assure patient has completed Phreesia prior to appointment; assist patient to complete Phreesia as needed.-Review patient charts 3-5 days before appointment to make sure everything is ready to go for appointment.-Assist with gathering outside records that relate to visit with the medical records team.-Communication with referring and/or primary care physician/provider on scheduled, canceled and/or rescheduled new patient appointment by fax, call or letter. -Send out letters/calls to new patients and/or Referring & Primary providers.-Schedule follow up appointments after new patient visits.-Review report of new appointments added each day to ensure charts are prepared prior to patient appointments.-Submit weekly New Patient Report to the Clinic Operations Team; will inform group of available, scheduled, canceled and no-showed appointment for the week along with if the patient was seen by all disciplines, made a return appt before leaving and comments about their appt.-Initiates check out process: Assist with scheduling PT/BH/Procedure/Follow up appointments with patients prior to leaving the clinic.-Fills in Front Desk/Scheduling when needed.-Assist and educate patients as needed with any questions/concerns.-Aids in clinic as needed.-Performs other duties as assigned to support the clinic.-Demonstrate general knowledge of clinic procedures and answer general questions related to them.
