COMMUNITY ADVOCACY – CHARMS 4, Inc. would provide advocacy and support by integrating families and individuals with resources within their communities. The goal is to help them identify services within any neighborhood that they choose to reside.PREVENTION SERVICES - Providing the traditional work of Case Management and focusing on ensuring access to Education, Treatment, Counseling and a series of Life Skills Trainings/Workshops (i.e., Self-Esteem, Decision Making, Conflict Resolution, Parenting, Personal Hygiene and much more). This would be through developing Individual Service Plans (ISP), for each participant served.MEDICAL – Referrals would be provided to all participants to visit with a healthcare provider. This provider would assist in, but is not limited to, educating participants on the importance of healthcare, updating immunizations, providing TB shots and any other general healthcare need for them and their children regardless of insurance.HOUSING – This service would provide participants in developing a Housing Plan. This would be administered via a housing assessment, credit counseling, a savings plan and giving/directing to various housing resources. This service would start the foundation for participants in repairing and restoring to full self-sufficiency. Each participant (with assistance) determines if there are any conspicuous barriers to permanent housing (i.e. previous evictions, negative credit reports or unpaid utility bills). Attempts will be made to eradicate any barriers revealed during the housing assessment. CHARMS4 will advocate with select property managers, landlords and real estate agents for affordable housing opportunities for all participants, regardless of their past history
