Locate housing for individuals who are mentally ill, homeless and assess ones need for appropriate placement. Work with Magellan Housing Department to address the need for community living and residential placement. Work with ACT Clinical Teams to place individuals in ACT Apartments. Conduct housing inspections. Staff various issues relating to recipients living in community living, ACT Housing, subsidized and independent housing. Certified Heart Assessor: Use Vulnerability Assessment Tool to assess recipients who are chronically homeless to determine eligibility for housing services for homeless adults. Network with various housing agencies such as ABC Housing, LDRC, Steele Commons, Native American Connections and other agencies. Work with Hom Inc, Biltmore Properties and Lifewell to address housing concerns for recipients who reside in subsidized housing. Coordinator and address the need for community living placement and NT-19 housing placement with Magellan Housing Staff. Monitor emergency transitional placement for recipients being discharge from inpatient settings, jail, etc. Connect with various landlords in the community to educated landlords about mental illness and rental availability. Maintain housing data to track the length of time that recipients housing applications being completed, being scheduled for a housing briefing and how long they have maintained housing. Provide clinical team housing specialist housing resources for low income, subsidized and assisted living. Provide housing tools that housing specialist can use to search housing websites such as Socialserve.com. Show less
