(4th year placement) Bass Coast Health gets referrals for many different clients of many different needs/ages. There is therefore a large range of clients that are served in this area, however in terms of OT majority of the clients are elderly clients. For the duration of my placement I was on the community OT team Mon-Wed and on the sub-acute OT team Thurs & Fri. Clinical Conditions: • Physically unable (due to age related factors) • Stroke • Tight hip flexors • Fractures (Shoulder, Hip, Knee) • Arthroscopy • Motorbike accident (#) • Recurrent falls • Functional decline Predominant Occupational issues: • Unable to complete self-care tasks: Dress self, Shower self, Toilet self. • Unable to mobilise to access bathroom, etc. • Unable to transfer in/out of shower safely (unbalanced with fear of falling) • Unable to walk long-distances x3 • Unable to transfer in/out of activity chair in aged care facility • Unable to lift leg over bath (shower over bath) • Unable to walk up steps to access house Age Groups: • Elderly 65+ • Adults (NDIS <65) Intervention Experience: Sub-acute OT: • Equipment prescription for discharge • Equipment education • Falls education • Energy conservation education • Documentation – file notes, discharge summaries, SMART goals • Referrals for post-acute in home care for discharge Community OT: • Recommended and sketched rails for showers • Recommended and sketched hand held shower hose • Prescription of scooters • Prescription of new activity chair • Prescription of manual wheelchair and wheelchair power assist • Recommended and provided bath board • Recommended and completed sketch for rail vs platform steps Assessments Administered: • Home assessment • Scooter assessment • PADL (shower) • Cognitive assessments (RUDAS, MOCA) • Cooking assessment Show less

The Victorian Department of Education currently do not employ Occupational Therapists within their school support service (SSS) teams, thus resulting in a gap of knowledge and clinical expertise. SSS staff require further knowledge on sensory processing deficits (SPD) in order to increase participation within the classroom for students experiencing SPD. Objective: - To increase CSSSS knowledge regarding sensory processing - To provide CSSSS with strategies that can be implemented to improve student learning outcomes To achieve these goals and objectives, two deliverables were developed: a presentation regarding SPD for Southern Melbourne SSS staff, and tip sheets including strategies to use for SPD. The evaluation of the project deliverables found the following outcomes: 1)Increased CSSSS awareness of SPD and knowledge of strategies to use within an inclusive school/classroom setting, 2) increased CSSSS awareness of when to refer to an OT. Overall, this project provides SSS staff with an understanding of SPD and strategies to implement when working with children experiencing SPD. Show less
