Experience

• As a Nurse Care Coordinator I am responsible for comprehensive work collaborating with patients interdisciplinary teams to deliver quality care management for our patients. As well as identify local resources and coordinate needs for chronic care management patients.



• Employed as a Hospice specialty Nurse to provide Palliative Care for End of Life stage patients with the use of WellSky EMR. As a Continous Care nurse with Good Shepherd Hospice, I provide peaceful passages for patients transitioning into end of life, as well as post-mortem care. I obtain admission data from patients newly admitted into our continous care services, and work with the interdisciplinary team (Team nurse, Chaplain, Social worker) to meet any needs of the patient and family.



• Employed as an Assisted Living Nurse to provide fulltime assisted care for patients in late adulthood. Participated with the patients interdisciplinary team to ensure necessary appointments were scheduled, transportation needs were met, correct and up to date medication orders were on file. Provided care to patients per my scope of practice according to the Florida Board of Nursing and the Policy Procedures of the Facility.



• Employed as a specialty Nurse in the Critical Care Step Down Unit, where I provided care to patients of all ages and with various disease processes with the use of Cerner and Epic EMR. Undersupervision of the charge nurse obtained admission data from new patients, discharged patients from the unit, transferred patients to ICU, administered medications, and provided patient care.
