VCS Transformation Systems Lead for West Sussex – Community Mental Health Services
-
Apr 2022 - Present
As part of the NHS Long Term Plan, across Sussex organisations including Sussex Partnership NHS Foundation Trust, Primary Care Networks, Voluntary, Community and Social Enterprise (VCSE), Social Care and Local Authorities have come together to form a Sussex Mental Health Collaborative within the Sussex Health & Care Partnership (SHCP). The SHCP Mental Health Collaborative will work to develop a mental health system that works for the people who use it, carers and for those who work within it. By putting those who use our services at the centre, we can strengthen the support we offer to help maintain emotional wellbeing and ensure better access to mental health services. These changes will better support people to access the services they need, when they need it in a way that works for them. This ambition includes developing a new workforce with lived experience, more flexible working and continuity of care, better integration with physical health and social care and enabling greater resilience through working with VCSE partners. The aim is not just to redesign care pathways but to develop a new ethos and culture that helps transform how we work together with partners.

Assistant Manager & Co-ordinator

I strategically coordinated shifts in a care on demand service with over 50 staff and 30 clients. That required using extremely organised and resourceful skills under a vast amount of pressure due to constant change. I managed a team of 12 support workers making sure they’re fully trained, competent and content within their roles which involved 9 clients we key-worked. I organised team meetings, attended regular reviews, psychiatric and medical appointments with expert professionalism and transparency. I was also part of the transitional team, attending assessments and working with other health professionals in helping vulnerable people transition into our service often from a secure unit. I was regularly on-call for the entire service giving support to clients and staff during crisis.

Community Care Coordinator

May 2014 - Aug 2014
Swanborough Services were a 24 hour Domiciliary Care agency that provided care for people with acquired brain injury, mental health illness and end of life care. I coordinated the Community Support Workers within the coastal team, creating rotas, facilitating meetings, orchestrating training sessions, undertaking interviews, attending assessments and reviews and undertaking staff supervisions/spot checks. I devised person centred care plans and was on-call for support to the entire company. I used public transport to travel to client’s homes offering support both physically and mentally.
