I managed a caseload of between 50-60 individuals with Severe and Persistant Mental Illnesses; while managing this case load I preformed duties including creating treatment plans, preforming annual assessments, making referrals to community and in-house resources, monitor the care individuals receive in Adult Foster Care homes, Semi-Independent Living Apartments, and independent homes. About 70% of my time was spent within the community making visits and preforming these duties.
