    • Community Health Manager
      • May 2019 - Jun 2023

Siloam Health is a faith-based, non-profit health care organization that serves the uninsured with a specialized focus on Nashville's immigrant and refugee populations. As the Community Health Manager, I lead a team of Community Health Workers (CHWs) who help to create health equity by addressing social determinants of health. I manage eight individuals who come from immigrant backgrounds and/or speak English as their second language. They represent three different programs that I manage: our Comprehensive Care program that serves patients with long-term diagnoses, our Rapid Response program that provides COVID support, and our Refugee Health Promotion Medical Case Management program that specially supports new arrivals. As the team's manager, I cultivate the staff's leadership abilities, decision-making skills, and confidence as they work with patients on their caseloads. By offering specialized one-on-one mentoring as well as weekly trainings, I keep their skills sharp while also challenging them to achieve new goals. This also includes serving as their first line of support during patient emergencies, requiring quick thinking while maintaining patient and team safety. Furthermore, with several of my team members being native Spanish speakers, I conduct much of my work entirely in Spanish. Show less
