Worked with pharmacists to improve Medicare plan STAR ratings by conducting adherence calls and other clinical quality improvement calls with members over the phone. After being hired the Medicare plan was able to achieve a five-star rating for the statin adherence metric for the first time, along with maintaining five-star ratings in diabetes and RAAS adherence metrics. Developed and improved many skills including motivational interviewing, working with an reporting data, coordinating and working with care providers, assisting with new intern/rotation student training, creating a monthly newsletter on practice updates for our pharmacy team and addressing gaps in care for our members.
