I make assessment and questions for post graduate medical personnels. an example is given below: Course: Pulmonary disease Topic Respiratory physiology and pulmonary symptoms Sub topic: Approach to pulmonary symptoms A 17-year old girl presented to you at the hospital with the complaint of morning headaches and dyspnea throughout the day; there is no history of cough; on examination, the patient is frequently sighing, and her pattern of breathing is erratic, the shape of her chest is elliptical, the chest x-ray is normal. What is your provisional diagnosis? Option 1:Acute asthma. Acute asthma is incorrect because the history is not compatible, there is no history of cough or wheeze, the patient has no symptoms of cough, and dyspnea in asthma usually presents with a diurnal variation with more breathlessness at night. Option 2: Interstitial fibrosis. Interstitial fibrosis is incorrect because the chest x-ray is normal; in interstitial fibrosis, the chest x-ray shows fibrosis opacities and trachea displacement due to shrinkage of lungs. Option 3:Psychogenic hyperventilation. Psychogenic hyperventilation is the correct answer; it is accompanied by morning headaches and dyspnea. Features that suggest psychogenic hyperventilation are inability to take a deep breath, frequent sighing, erratic breathing, and difficulty in holding breath. Option 4:Lobar collapse. Lobar collapse is incorrect because there should be a homogeneous opacity on the chest x-ray i case of lobar collapse. Option 5:Chronic obstructive pulmonary disorder. Chronic obstructive pulmonary disorder is incorrect because the history does not correspond with it; the patient should be above 40 years of age, the shape of the chest is abnormal, and there is a history of productive cough in chronic obstructive pulmonary disorders.
