      Responsibilities include: •Developing and providing direct patient care and integrated pharmacy services for PICU patients •Working closely with nurses, intensivists, surgeons, hospitalists, and other prescribers to optimize safe and effective drug therapy •Assisting in the creation of PICU, pharmacy, and hospital policies and protocols •Assisting in the precepting of pharmacy students/residents while on their general pediatrics and ICU rotation •Monitoring drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication and pharmacokinetic and pharmacodynamic drug interactions •Performing renal and hepatic dosing adjustments •Optimizing patient drug regimens (IV to PO conversions, decreasing sedation when appropriate, weaning off sedation/narcotics, discontinuing inappropriate medications) •Educating medical residents and nursing staff about pharmacokinetic differences that exist between neonates, infants, children, and adults •Educating medical residents and nursing staff about pharmacokinetic differences that affect drug dosing when patients are placed on Extracorporeal Membrane Oxygenation (ECMO), therapeutic hypothermia, photopheresis, and plasmapheresis. •Educating medical providers on amount of drug clearance brought about through ECMO, plasma exchange/pheresis, dialysis, and therapeutic hypothermia. •Providing clinical pharmaceutical services such as aiding the medical team with proper selection of medications, streamlining antibiotic/antifungal therapy based on cultures and sensitivities, & answering nurses’ therapy related questions on medication administration and compatibility •Performing medication reconciliation of all new admissions into the PICU • Promoting the use of the formulary by converting non-formulary drugs to those listed on the formulary (when possible), but also coordinating procurement of non-formulary drugs if needed Show less
