      MeHAF Thriving in Place is a grant funded program helping people with chronic health conditions maintain their health so they can remain living in their home and community. With the Aroostook Area Agency on Aging, the project director works in collaboration other area providers on innovative approaches to health care to enhance your health and wellness so you can thrive in place and stay in your own home. Our mission is to identify what is working, identify what needs to be improved and assist people so they can remain in their homes. Research shows that over 90% of Mainers want to stay in their own home and community, but that current resources to help them live safely and healthy are not sufficient and not always effective. HOW CAN WE HELP YOU? We are working with hospitals, medical and home health providers, emergency and ambulance services, colleges, high schools, churches and other community service organizations, to develop volunteer and other service opportunities to enhance and improve the quality of life of MeHAF participants and the home-bound and aging population. We can REFER you to COUNTY WIDE Coordinated community supports for screening, self-management, education and check-in services with our community partners. We can MONITOR your services and FOLLOW-UP through a centralized information system to provide the best support. We can help you with KEY ISSUES like medical transportation, home care services, in-home supports, meals on wheels and adult day care for caregiver respite. Show less

      Serve the Community *** I coordinate Volunteers who serve at Meals on Wheels with delivery to homebound seniors, prepare Hot Meals at Senior Centers, or Visit the Homebound; they also Transport Seniors to their Medical Appointments, Radiation Treatments or Become a Friendly Visitor (in-home) ~ giving partner care-givers a small break once or twice a month or weekly; Work with Veterans Services, volunteer at Thrift Shops and Food Pantries; or they educate seniors about Money Management, Medicare Part D, simple Tax Preparation and help to identify Fraud and Scam situations. Also, many of our homebound seniors make Knitted Goods for our 3 Regional Craft Fairs for fundraising efforts and then provide the remainders through the Warm Hands & Feet Initiative in County Schools and other non-profit agencies as well as making regular Telephone "Reassurance" calls. Others serve on the Advisory Council or with the American Red Cross Disaster Relief Service. This is a Grant Funded program that Serves High Impact Areas.
