My employment with Kindred at Home includes case managing a specific area within the company. On average, I manage 55 to 70 patients with the assistance of one LPN. I am responsible for admissions and discharges to home, health as well as reassessments for continued care. I coordinate care between physicians for patients and refer patients to different MDs when needed. I often teach/manage IV infusions in the home setting, manage/perform wound care of all sorts, and care for those recently discharged from the hospital with various conditions (acute chf, acute copd, covid, covid, pna, s/p surgery- cabg, tarv, joint replacements, and amputations to name a few). At DMH, I worked full-time in surgery and prn on the med-surg floor. In surgery, I complete pre- op assessments with patients verifying their health history, their scheduled procedure, and provide pre-op instructions. On the day of surgery, I brought patients back and set them up for surgery (verifying patient, allergies, connecting to monitors, starting IVs, provided pre-op medications). Once surgery is completed, I provided post anesthesia care by monitoring vitals and responsiveness. Once patients were ready to be discharge I will provide discharge education with follow up information. On the floor, I would take responsibility for up to 10 patients. During the shift, I will do an initial assessment, pass medication, chart every two hours, and provide any ADLs needed. Show less

At GRCF I was the RN on duty for nights. Typically, I worked with one to two LPNs, one who worked in the pharmacy and the other who would work with me. I was responsible for all the lab draws, insulin, dressing changes, charting on any infirmary or SHOS patients, triage, sharp counts, and all medical/psych emergencies that may come in.
