      Lexham House is run on behalf of the Richmond Psychological Institute and is a thirteen bedroom property which offers a range of 12, 24 or 36 week aftercare programs.       Client group - primarily substance use. My role as an apprentice consisted of working with a wide range of service users who had accessed our services through either self-referral, Drr’s, outreach and hostel satellite. During this time I was able to work with two project specific case loads which for a time amounted to forty three clients I found this very challenging however I was able commit to this work load and work closely with both team leaders utilising supervision and Case management to ensure all service users received the best care and support suitable towards many successful outcomes achieved by each service user with the support and guidance from the Project, Colleagues and Myself. Whilst working closely with service users it was required to work in conjunction with partner agencies within the borough and beyond to ascertain the most suitable path for which the service user can best succeed in their journey of recovery. I found an important part of this process was supporting and accompanying the service user to appointments such as assessment processes at borough prescribing services, moving to new accommodation and working closely with fellow key workers within these agencies for a smooth transition on behalf of the service user. I also worked closely with Air Football Promotion, outreach and Saturday sports session which targeted primarily young men with possible involvement in street crime, drug use and connections to gangs in order to use sports and particularly Football to build confidence and give focus towards a positive outlook which has led to many accessing college, training and employment. I have led groups such as stimulant group and Cannabis group and extensively worked in a satellite capacity within hostels throughout the borough Show less
