I played a role in delivering emotional, personal care and support for vulnerable adults daily, who are known for presenting challenging behaviour. I was involved in providing quality care within a specialist residential setting and followed person-centred care plans. I took part supporting individuals 1:1, 2:1 and 3:1 where necessary. Individuals I supported have complex needs, many of whom experienced community placement breakdowns and/or prolonged hospital admissions due to high complexity and risks relating to their behaviours. Many have learning disabilities, autism, genetic brain conditions, mental health issues. I supported them within a specialist residential home and out in the community. I became a peer mentor for Gower College Swansea after having training with Hafal Wales, a mental health charity. I was a person who was on hand to listen whenever someone needed to talk about personal issues. This was a confidential service. I supported and guided students, and when necessary, directed them to the appropriate services if in extreme circumstances, understanding my limitations as a student peer mentor. I wrote reports regarding the situations for records, whilst maintaining confidentiality and arranged appropriate activities for self evaluation. Show less
