St. Joseph Medical Center, Baltimore, Maryland Neonatal Intensive Care Unit For the past seventeen years, I have worked under the direction of Drs. Michael Langbaum and Marilyn Bennett in the role of Certified Neonatal Nurse Practitioner. During this time, I have practiced in the comprehensive medical management of critically ill neonates. Within a 12 hour shift I provide complete medical care to a caseload of approximately 5-18 moderately to severely ill term, preterm and very low birth weight newborn infants. Some of the responsibilities involved in this role include fluid and electrolyte management, initiation and titration of vasoactive medications, nutritional support, surfactant administration, attendance at high-risk deliveries as well as pediatric emergencies in the Emergency Department and the Pediatric Inpatient Unit. Some skills necessary in this position include daily physical assessment, endotracheal intubation, umbilical vessel catheterization, arterial and venipuncture, percutaneous central line placement and chest tube placement. Frequently, I have had the opportunity to care for infants with severe respiratory failure including severe RDS, PIE and air leak syndrome, PPHN and meconium aspiration syndrome who have required high frequency oscillatory ventilation with the SensorMedics High Frequency Oscillatory Ventilator. We also utilize Inhaled Nitric Oxide for treatment of PPHN. Also in this role, I have been responsible for performing discharge physical examinations and preparing discharge summaries for the chart. Additionally, this role has involved working with the parents and families of critically ill neonates as they progress through the infants’ hospitalization.

Union Hospital of Cecil County, Elkton, MarylandMaternal Infant Center For twenty-one months I functioned in the role of Neonatal Nurse Practitioner in the Maternal Infant Center. The purpose of this role was to attend all deliveries and provide immediate resuscitative measures to those newborns as well as monitoring the well-being of all newborns in the unit. In the event that a newborn required prolonged respiratory support, the NNP was responsible for stabilizing the infant and maintaining support until such time as the Neonatal Transport Team was available to transport the infant to a tertiary care center. Some of the responsibilities included review of maternal history, immediate assessment and stabilization of the newborn, admission and discharge physical assessments, ordering and obtaining blood work via arterial and venipuncture, interpretation of labwork, insertion of peripheral IVs, ordering antibiotic therapy and ordering and maintaining respiratory support including endotracheal intubation if needed. In this position, one needed to collaborate with Obstetricians, Pediatricians and nursing staff. Also, providing a source of education about sick newborns to the Labor and Delivery nursing staff was an important aspect of this position. Another responsibility of the NNP in this environment was to provide education to the parents regarding their newborn infants.

The Johns Hopkins Bayview Medical Center, Baltimore, Maryland Neonatal Intensive Care Unit Prior to my present employment, I spent seven years at Johns Hopkins Bayview Medical Center under the direction of Drs. Fabien Eyal, Gad Alpan and their staff in the role of Student and then Certified Neonatal Nurse Practitioner. During that time, I participated in the comprehensive medical management of moderately to severely ill term, pre-term and very low birth weight newborns. The responsibilities that were involved in this role were the same as those in my current position including fluid and electrolyte management, ventilator management, initiation and titration of vasoactive medications, nutritional support, surfactant administration, attendance at high-risk deliveries as well as pediatric emergencies in the Emergency Department. Some of the skills which were necessary to function in this position included daily physical assessments, endotracheal intubation, umbilical vessel catheterization, lumbar puncture, arterial and venipuncture, percutaneous central line placement and chest tube placement. Frequently, I had the opportunity to care for infants with severe respiratory failure including severe RDS, PIE and air leak syndromes, PPHN and meconium aspiration syndrome who required high frequency ventilation with the Bunnell Life Pulse Jet Ventilator and the SensorMedics High Frequency Oscillatory Ventilator. In this role, I was responsible for performing discharge physical examinations and preparing discharge summaries for the chart. Additionally, this role involved working with families of critically ill neonates as they progressed through their infants’ hospitalization. This included bedside teaching as well as formal family conferences.
