This service provides a Harm Reduction and Person-Centred Model of Service. Within this role I was providing care and support for men living with mental health and addictions in their lives to foster the support needed to promote independence. Provided individualised case management for men accessing the program and assisted individuals connecting with other agencies. Other responsibilities included: Walk arounds on a random schedule ensuring outside doors are locked and secured Ensure only tenants and supervised guests are in the building Mediate any potential problems between tenants and guests, or any outside agencies Intervention and support services for tenants as situations arise Maintain communication log and write up incident reports when required In partnership with another colleague, we also developed and implemented a Games Night Program into the service. This Program allowed for conversation with service users which may not have happened on its own. We were able to use this service to discuss issues that would usually be covered within a men's group. Using the game night we were able to present these issues in a less threatening and interfering way.

The goal of CARS staff is to assist individuals with developmental disabilities to become more independent through skill acquisition, empowerment and full inclusion in society, thus improving their quality of life. The program focuses on ‘person centered planning’ – people supporting people – based on the individuals needs. Some of my main responsibilities as a Frontline Community Support Worker was aiding clients with physical, mental or developmental disability to help live their lives as independently and healthily as possible. Working on a one to one basis, I helped to assist clients to reach their set goals, increase level of community access and live as valued members of their community. Facilitated support as part of a team to provide and coordinate individualised care plans and acted as an advocate for clients whenever needed.
