  * Evaluation, treatment and reassessment of patients with complex medical needs with regards to physical therapy outcomes. * Development of plan of care based on individual patient needs, and presentations, and amendment of plan of care based on evolving medical and physical presentation * Interaction with a multidisciplinary team, including PTs, PTAs, OTs, STs, nurses, CNAs, neurologists, cardiologists, pulmonologists, hospitalists, case managers, and social workers to provide optimum outcomes for patients * Provide training to support staff and healthcare professionals regarding transfers, positioning, gait training, body mechanics, management of mobility aids, donning/doffing of orthotics and prosthetics. * Monitoring and updating of standards of practice to ensure safe and effective workflow which maximizes efficiency and effectiveness of patient care. * Patient population consists of a variety of high needs patients with complex and evolving medical conditions which can include any number of the following: Acute/chronic respiratory failure, CVA, TBI, MI, AKI, CHF, Guillain-barre, SCI, complex wound care, as well as a variety of post-surgical complications ranging from ileostomy to craniotomy. * Patients are seen multiple times per week with an emphasis on improving functional mobility, with treatments for improved bed mobility, transfers ambulation, balance, and endurance. Treatments also emphasize evidence-based practice and integration of continuing education. * Pt outcomes are directed towards discharge to SNF or Acute rehabilitation clinics, with occasional patients who are suitable to discharge to home with home health assistance.
