Oct 2019 - Present
It was my responsibility to support individuals with their medication, home care, shopping, cooking, banking, attending appointments and social/community activities. I was a key worker which involved keeping individuals care plans and medication folders up to date. I monitored their goals and outcomes monthly and took part in their reviews. I also had additional responsibilities in health & safety, participation and evacuation. I had choking training, first aid training, challenging behaviour training, fire safety training, core values and adult support and protection training. I worked through the entire COVID-19 pandemic and therefore also done PPE and infection control e-learning. I done sleepovers every week where I was lone working. I was responsible for collecting medication from the pharmacy each week and signing it in. I was responsible for the supported individual’s money, some of whom are in trust, doing safe checks in the morning and petty cash returns at the end of each week. I have experience creating MAR sheets, medication information records and hospital passports. I often liaised with the supported individuals care teams and other health professionals. I done weekly health and safety checks and deep cleans in each supported individual’s flat. When new staff joined the team, they shadowed me for two weeks and it is my shared responsibility to show them how to do everything and help them settle in.
