• Providing direct support (one on one) to clients with a diagnosed psychiatric disability. • Providing positive home and community based experiences and care for clients who suffer from a mental illness as per their Individual Support Programs. • Ensuring that medical and therapeutic interventions are current and administered in a way that benefits the client and their wellbeing. • Enhancing clients skills based in areas that promote and increase independence and wellbeing but not exclusive to budgeting, day to day planning, housekeeping, goal setting and personal growth • Supporting the client in sustaining a living situation that is affordable, comfortable safe and secure which promotes the notion of the client as a independent and able member of the Aboriginal & Torres Strait Islander community. • Assisting clients to attain goals which are outlined in their Individual Support Plans and / or building on skills are identified by the family. • Providing feedback to family as request/as necessary • Providing feedback to Co-ordinator / Manager as required • Providing feedback to Case Manager as needed. • Participating in staff meetings, development opportunities and general service meetings are requested. • Completing required case notes at the end of each shift and ensuring these are signed by clients and completing in a timely manner. • Completing incident reports when required. • Participating in supervision and performance reviews in accordance with Wuchopperen requirements. • Other duties as required, within my skills, experience and capacity • Chairing program meetings • Minute taking for program meetings.
