Rotated around multiple wards (covering various disease areas) and gained competence in medicines reconcilliation. Performed interventions on patients medication when more appropriate therapy should be considered. Spent a large portion of time within the dispensary which included accuracy/clinical checking, dispensing, and labelling. Assisted for four weeks in Medicines Information, providing evidence based answers to queries from the public and healthcare professionals. Conducted several audits, including appropriate upkeep of Controlled Drug registers on surgical wards and appropriate prescribing of low molecular weight heparins across the trust. Rotated in the ADU, using aseptic technique to manipulate intravenous drug additives as well as checking completed products for particulates, etc. Developed guidelines for the prescribing of Esomeprazole and Rabeprazole which were submitted to the Area Prescribing Committee for review.

Shadowed pharmacists around various wards to grasp a greater understanding of their role within a hospital based setting. Observed how pharmacists communicate with both patients and other healthcare professionals. Spent time within the dispensary, performing the standard procedure for dispensing outpatient items. Learnt the role of the medicines information department in providing informed advice to healthcare professionals regarding a range of problems, from contraindications to adverse events. Attended a seminar on the correct use of various types of inhalers and the problems that can arise from incorrect use.

Used skills and knowledge obtained from pharmacy practice teaching to dispense a large range of prescriptions, adhering to the standard operating procedures as set out by Tesco. Present in patient counselling sessions including the dispensing of emergency hormonal contraception and blood pressure measurements. Provided advice to customers on over the counter medicines including herbal remedies such as St John’s Wort.
