  I serve as the point of contact for the Department of Children and Families for all ABC Home Visiting program referrals for all three counties in Northeast Kansas (Wyandotte, Douglas, and Leavenworth). As the Community Navigator, I screen and accept referrals and provide the DCF referring worker with the ABC Home Visitor contact. Other job duties include but are not limited to: • Reporting referrals to the Assistant Director of Health and Screening Services.• Supporting the Connections centralized intake and referral system.• Meet with families desiring assistance and use interviewing techniques/formal assessments to determine the program which can most effectively meet the families’ needs.• Assist the family in accessing community resources that are needed immediately.
