Primary care is the first point of contact a person has with the health system—the point where people receive care for most of their everyday health needs. Primary care is typically provided by: Family physicians Nurses Dietitians Mental health professionals Pharmacists Therapists Family physicians and associated health personnel routinely provide primary care services within Primary Care Networks. Family physicians are often the first step for patients. The Northwest PCN is a partnership of the physicians from the Fort Vermilion Associate Medical Clinic, the High Level Medical Clinic and the La Crete Medical Clinic. The mission of the Northwest PCN is to increase access to primary medical services for the residents of northwest Alberta. A number of programs have been initiated by the Northwest PCN to increase access; a few of which are: Chronic Disease Management, Education and Support This program provides education and support for individuals diagnosed with a chronic condition. Complex Needs Programming This program is available to anyone with complex health needs who is experiencing difficulty accessing housing, financial or other programs. The Complex Needs Program also offers assistance with referrals and patient advocacy. Perinatal Programming This program provides support and education during and after pregnancy. The Perinatal Program also assists women and their families in navigating the health care system; helping them access the services and care givers they require. Visiting Specialists The Northwest PCN in partnership with Alberta Health Services has arranged for a number of medical specialists to visit northwest Alberta on a regular basis. This program allows residents to access specialist appointments without the need to travel south. Show less
