      • Nov 2016 - Feb 2021

The clinical case management position is an office-based role that involves the review of the clinical aspects of care1) Evaluation of health summaries and the ACAT National Screening and Assessment Form. Based on this review, we provide recommendations for in-home care to the case managers in the field. 2) Provide a triage and advice point of contact for the care workers and Clinicians in the field. eg they will call for advice if a client has a fall, medication administration concern, change in health and wellness etc. 3) Develop assessment charts for use by clinicians in the community. Provide education for care workers when requested by their manager. 4) Collate and evaluate data in relation to clinical incidents5) Update the individual care plan to incorporate instructions to address clinical needs in consultation with the client. 6) Liaise with the multidisciplinary team, including the general practitioner to ensure that each client receives best practice holistic careThe telehealth component is also an office-based role. This program assists clients with chronic diseases such as diabetes, hypertension, emphysema and heart disease. We provide a tablet with blue tooth monitoring devices (glucometer, blood pressure machine, oximeter, scales) so that they can submit readings daily for our review. The monitored readings appear on our desktop app within minutes of monitoring, if the readings are deviating from their normal range, we make contact with the client to discuss the appropriate course of action. Health promotion and health literacy advice is our main focus.
