As a case manager at Santa Cruz AIDS Project (SCAP) -a part of Encompass Community Services, I provide compassionate and nonjudgemental emotional support, service plan development, referrals to providers in the community, and oversee or provide the following support services:Benefits Advocacy. I assist clients towards receiving, maintaining, or reinstating their benefits. People with HIV/AIDS may qualify for benefits offered at the county, state and federal level. These benefits may include: Social Security, Medi-Cal, MediCare, AIDS Drug assistance Program (ADAP), Early Intervention Services (EIS), State Disability, Cal-Works, General Assistance and more.Financial Assistance. I advocate for clients by understanding and utilizing funding sources like Housing Opportunities for People With AIDS (HOPWA) and the Ryan White CARE Act. These programs provide financial assistance towards rent, utilities, transportation to medical appointments, and extra nutritional support.Housing Support. I work with clients to find and maintain stable housing. This includes overseeing residents living in supportive housing for SCAP clients and working with outside agencies like the Homeless Service Center (HSC) and Housing Authority of the County of Santa Cruz. Food and Nutritional Assistance. Located at SCAP's main office, a food pantry is available along with information about nutrition and healthy recipes. Promoting Wellness. SCAP is a strong supporter of the integration of mind, body, and spirit so that one has a sense of balance and harmony in life. Wellness promotes self-care, self-understanding and connects one's life to the larger world. HIV/HCP Rapid Testing. I am a certified HIV and Hepatitis C test counselor and I stay updated on the latest information related to sexual health, harm reduction strategies, and fun and empowering ways to communicate about sex!
