Responsible for daily oversight as well as short and long-term improvement in operations quality and efficiency for the Upstate Adult Medicine Clinic. Achievements to date include: * Implementing new policies and procedures aimed at closing the referral loop in 90 days or less to eliminate backlog and ensure process efficiency and timely patient care. Since implementation the number of outstanding/active referrals has dropped from over 1000 to under 500 (daily count), in a clinic that generates 170-250 unique referrals per week. * Worked with IMT to develop an Adult Med Social Work queue, orders, and SW pool to improve the tracking of utilization and need for social work in the adult medicine department. * Collaboration with embedded pharmacy and behavioral health teams. * Established in-room check-out process for patients requiring translation services * Collaboration with payors to identify gaps in care, creating opportunity for improved patient outcomes and additional revenue for the clinic. * Ensuring proper training and clear expectations for staff. * Ongoing QI projects aimed at improving efficiency and effectiveness of clinic operations and patient care. Interim Project Manager for PCMH - leading the ambulatory-wide transition to 2017 and NYS standards, and successful annual reporting for ongoing recognition. Project Lead for successful CNYCC Innovations Fund Grant application. Now Program Lead, including oversight of program development and implementation, coordination with community partners, contracting, quarterly reporting, and budget management and oversight.
