  Working with Tonbridge PCN and accepting GP referrals for children with neurodevelopmental and/or mental health needs. Supporting the families to improve the outcome for the child for health, education, social and emotional needs by collaborating with professionals such as CYPMHS, community paediatrics or schools. Referring and signposting the child to prevent harm and minimise delay. This is not a time-limited role and can be utilised to support pre- and post-diagnoses, such as autism or ADHD.
