I am lead physician for a primary care practice which provides service for all ages. I am also board certified in Hospice and Palliative care, so this practice provides long term palliative care services. We have a nurse coordinator who helps manage those with chronic disease through care in other settings such as hospital, assisted living, home health, and hospice services. We focus on long term primary care services and provide after hours access by phone, email (through secure portal), and some after hours care to our established patients. We provide acute care, preventive care services, wellness services, and excellent chronic care management. Our practice is part of a primary care based Accountable Care Organization, Cumberland Center for Healthcare Innovation.

I worked with others in the application for the advanced payment program (APP) offered by the CMS Center for Innovation, and the Medicare Shared Savings Program (MSSP). Our contract start date with CMS was July 1, 2011. As the Chief Medical Officer, I chair the clinical transformation council, and lead the coordination of clinical transformation and clinical quality for this network of 29 medical practices, 36 physicians in 14 counties of the Upper Cumberland Area of Tennessee.

Started as a solo practitioner, providing care from obstetrics to geriatrics. OB care included C-sections. Procedures included colonoscopy and EGDs. I cared for hospitalized patients, including ICU patients and ER coverage. This practice was initiated in 1991 with electronic medical records. Mid-Level providers (Nurse Practitioners and Physician Assistants) were added in 1994. We became a corporation , Cumberland Family Care (CFC) in 1998. CFCstarted a diabetic education center and drug and alcohol treatment program which also addressed other behaviors such as obesity/eating disorders and smoking cessation. This practice was a pioneering practice in adopting components of the Patient Centered Medical Home. CFC facilitated shared medical visit (group visits) for 12 years.
