  Provided 1 on 1 support to people living with HIV/AIDS in the Saginaw area who have dropped out of medical care by identifying barriers that led them to drop away from care, remove those barriers, and support the client so they can re-enter and successfully navigate the healthcare system. Maintain current, and establish new, HIV testing locations in the Saginaw area to provide free HIV testing to identify persons who are infected with HIV so that we can assist them in connecting with appropriate medical care. Maintained positive relationships with local hospitals, health departments, STD/HIV clinics, doctor’s offices and health and human care service agencies to facilitate the referral process of HIV+ persons who have dropped out of medical care. Planned and hosted focus group of HIV+ persons who have recently re-entered medical care to determine the reasons why they dropped away from care, assess the effectiveness of the navigation program, and to use this information for future planning of the navigation program.
