Transition Aged Youth Wraparound Clinician

I provided client and family-centered, trauma-informed clinical treatment for transitional aged youth individuals, and their natural supports with mental health needs. I provided intensive home based services as well as intensive care coordination services for transitional aged youth clients in community based settings. In this role I coordinated care between clients, their families, and a variety of providers including those from Human Services Agency, Juvenile Probation Department, School Districts, Transitional Housing Programs, and others. I also developed treatment service plans based on the comprehensive assessment in collaboration with the client and their family. Throughout my time at Seneca I also was a mentor for multiple newer staff members, and assisted in training.

As a Clinician at Victor Community Support Services, I established and implemented quality, trauma-informed clinical treatment and case management for children and adolescents with mental health needs. I provided individual and family therapy, as well as collaterals with parents/guardians, social workers, and other individuals in community-based settings. In this role, I conducted comprehensive mental health assessments on assigned cases to determine the need for mental health services, and assess for a diagnosis if applicable. Additionally, I developed treatment services plans based on the comprehensive assessment in collaboration with the client and family, and maintained DBH and Medi-Cal documentation for a full caseload in a timely manner.
