  Designed and Implemented a low cost NP-led Heart Failure Disease Management (HFDMP) at JBVA. Since it's inception in Oct 2010, the program has grown in multi-dimentions using the available resources. As a result of the out-patient clinic and Project Red, the re-admssion rate at JBVA has improved from >25% to below 17% .
