  Outreach assessment of clients living with chronic disease who are at high risk of avoidable presentations to hospital. Clients are assessed to ascertain need for health education to improve self care management and the need for medical specialist services. Clients are also assessed for the need for in home and community supports for both the client and their carer/s.
