• Provide rehabilitation services, contribute to and develop treatment plans and manages crises for all residential clients.• Perform case management duties for clients such as; medication compliance, schedule and attend doctor appointments with client, budgeting skills, money management and referrals• Log and progress all activities that are client based in the CIS system that are billable by the State• Handles referrals and discharges for residential services• Assist clients in obtaining/maintaining entitlements such as SSI and SSDI• Read communication logs and progress notes daily and respond accordingly• Provide support to clients including apartment inspections, community resources, community support and modeling appropriate behaviors in the community• Daily chart all pertinent information and observations regarding interactions, behaviors, mood of group and maintaining client charts• Provide instructions/assistance in daily living skills • Obtain and maintain community resources support, such as; attending education programs and school, volunteer and job opportunities and using local community resources• Handle applications and paperwork necessary for the Department of Human Services• Administer medication and maintain Medication Observation Reports and assist clients to monitor medication• Contact appropriate authorities in an emergency• Run therapeutic groups, plan and participate in scheduled/unscheduled activities• Meet with supervisor and attend staff, in-service and team meetings as required Show less
