  Currently I am part of the intake team who assists in getting members onto services. A functional assessment is completed and services are determined by their needs. Case manager is responsible for making sure members complete the appropriate paperwork. Once the member is approved the case manager coordinates services and passes case off to ongoing. I have also been apart of ongoing and have managed members cases. Some duties are to complete regular contacts, find placement in facilities, and completing assessments.
