Aging Care Manager II (civil service) **August 2010-October 2010 the same duties were performed under a contracted position with Family Services Association of Bucks County** • Provide comprehensive service coordination in programs funded by various sources including Medicaid (DPW) to facilitate in home care through various programs such as Aging Waiver, and Options • Assess ability to complete Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) • Provide intervention as an Adult Protective Service Worker when needed. • Accurately take Reports of Need for Adults over 60 at imminent risk of death or serious bodily injury for distribution to the Protective Services Staff. • Connect over 60 Bucks County residents with resources provided through other agencies in the community such as the Bucks County Housing Authority, Mental Health, Bucks County Transport etc • Coordinate care with various providers of in home services and assess the changing needs of those served. • Efficiently implement changes to prevent endangerment of consumers • Investigate reports of elder abuse, neglect, and exploitation as outlined in the Adult Protective Services Act. • Maintain the confidentiality of consumers before, during, and after services are provided • Assess the appropriate level of care as it pertains to the placement of a consumer in a skilled care facility or other community program • Obtain placement in skilled nursing facilities as appropriate • Coordinate care plans with other services such as hospice to ensure consumer needs are adequately met • Assist in the apprehension of required household services such as exterminators, electricians, and medical equipment to maintain a safe home environment for consumers • Maintain accurate record of all contacts made and actions taken which pertain to a specific consumer or that consumers care
