  Professional Provider Organisation Services - PPO Serve - is Brian Ruff's Value Based Care business initiative to help create local, effective Multi Disciplinary Teams called Integrated Clinical Consortia. These businesses are owned by the clinicians and they provide standard clinical products under a shared brand, in multiple locations nationwide. The primary healthcare product is The Value Care Team (VCT). The consortia are organised to work as patient centered teams with shared patients, income and accountability, using shared clinical IT for their workflow. Their customers are people across the spectrum; medical scheme members; self funded patients or State funded. The teams never use Fee for Service, rather they are global team fees with a significant 'value based care' portion in contracts. PPO Serve currently has 2 products: - The Value Care Team does population medicine - the GP Care Cell does HIV enrollment and management, funded by the State. It now also does PrEP. PPO Serve has developed a clinical IT system - the Intelligent Care System (ICS) to support the Value Based Care work. Its design is patient centric, and it has 3 functions: i. displays a key patient summary derived from past claims ii. collects detailed diagnostic severity, disability and social determinants information (done in our model by Care Coordinators) for a comprehensive view of the patient challenges; and iii) links to the diagnoses Care Plans with proactive tasks for the Team to optimise outcomes. PPO Serve is exempted from needing to register as 'Managed Care Organisation' because neither it nor the consortia take financial risk from the contracted Scheme.
